Cyborg, et pas androïde : un androïde est une machine à part entière, humaine seulement d'apparence. Un cyborg est une créature issue de la fusion entre la machine et un organisme humain. Le major Kusanagi possède encore son cerveau, enfermé dans une boîte crânienne en métal renforcé, haute protection, et enchâssé dans son corps artificiel. C'est tout ce qui la rattache à son humanité d'origine.

La trame principale du manga, et la trame du film, raconte la traque d'un cybercriminel connu sous le nom de « Marionnettiste » (Puppet Master). Ce cybercriminel prend le contrôle de l'esprit d'un humain par l'intermédiaire du Réseau Numérique mondial, descendant d'internet.
